Experienced Professional Registered Nurse to provide an in-home comprehensive physical head to toe assessment and home safety assessment to avoid unwanted falls, injuries and hospitalizations. Professional nursing care you deserve. Licensed and Insured.
Chronic Disease Management
Here to help you manage your symptoms to avoid unnecessary hospitalizations and improve your quality of life. Your personal home care nurse will work with you and your physicians to create a personalized treatment plan with appropriate interventions such as in home CNA care, physical therapy, occupational therapy, speech therapy, medical social worker, advanced care planning, transportation and meal coordination.
Home Maintenance and Safety Program
Our monthly maintenance program provides comfort and safety by insuring routine smoke alarm battery changes, changing light bulbs & air filters, monitoring salt levels in your water softeners, and pressure washing outdoor areas to remove dangerous mildew, mold and algae from walk areas. Other services such as moving large items and haulaway of unwanted items are available on request.
